If you have the user option "Print Addresses in International Format" enabled, you may be impacted by a bug in the current version of DGIS that results in the Shipper's address printing in the Consignee address field on the Shipper's Declaration form:
We are aware of the issue and are working on a permanent fix, but in the meantime you will need to disable the user option in order for the Consignee address to print on the form. To do so, go to the Administration tab, select User Options on the left hand side, and then find the option under the Report Options section:
You will need to disable the user option and click 'Save Options' in the upper right. You should now see the correct addresses when printing any new forms. If you are re-printing an existing one, you may need to click the blue 'Regenerate Reports' button in the upper right corner of the preview window in order for the change to reflect.
If having addresses in the domestic format (city before postal code) is acceptable, then there's nothing further you need to do. If you do require the international format, you can accomplish this with the user option disabled by swapping the city and postal code fields when filling out the form (putting the city name in the postal code field and vice versa).
